Long Term Service and Support Providers Rate Change Notice

Superior HealthPlan rate changes effective 9/1/2011 for the following services:

Personal Attendant Care Services – Procedure Code S5125, Modifiers U3 U3
(STAR+PLUS Waiver), non rate enhanced= $11.20 per unit. For providers who participate in the Attendant Care Rate Enhancement the new rate is $12.20 per unit.

The new rates will be paid for all claims filed for dates of service September 1, 2011 and forward. Please note that claims will be paid at the lesser of allowed amount or billed charges. If charges billed are less than the contracted rate, the billed amount will be paid. All clean claim filing requirements & timely filing guidelines remain in place.
