Superior is expanding our provider networks statewide for the Texas Health and Human Services (HHSC) Request for Proposal (RFP) Number 529-12-0002. We’d like for you to join us! We partner with physicians, hospitals and other providers to ensure that each member gets the right care, at the right time, in the right setting. In this section you will find the contract request form.

To Become a Texas Medicaid Provider you must attest with TMHP. For more information please visit TMHP Provider Enrollment. You cannot be reimbursed for Medicaid services without attesting with TMHP.

FOSTER CARE NOTE: If you are interested in joining the STAR Health provider network for foster care as a dental, vision, or behavioral health provider, please click here. PCPs and all other specialties may request a STAR Health contract by completing the form below.
