To Become a Texas Medicaid Provider you must attest with TMHP. For more information please visit TMHP Provider Enrollment. You cannot be reimbursed for Medicaid services without attesting with TMHP.

FOSTER CARE NOTE: If you are interested in joining the STAR Health provider network for foster care as a dental, vision, or behavioral health provider, please click here. PCPs and all other specialties may request a STAR Health contract by completing the Superior HealthPlan Contract Request Form.
