Effective September 1, 2023: New Behavioral Health Provider Form Available in Health Passport

Superior HealthPlan has added a new form in support of the contractual requirement for behavioral health providers treating Superior STAR Health (foster care) members. The purpose of this form is to provide initial and monthly summaries to be displayed in Health Passport. Providers must use the new form beginning September 1, 2023.

The new form is titled “Initial and Monthly Behavioral Health Assessment” and can be accessed directly within Health Passport by navigating to the Assessments module and clicking on the Submit Forms tab.

The new form can also be accessed on Superior’s Foster Care Important Forms webpage.
