There are many goals of our Stay Healthy programs and services. These include to provide you with information, access to care and a Care Manager for support.

  • Disease Management
    • Our disease management programs offer advice, health-care tips and coaching. In this section you will find information about our programs for asthmadiabetes and more.
  • Care Management and Service Coordination
    • We want you and your children to grow up healthy and stay healthy. Our Care Managers and Service Coordinators can help you find community resources and services you may be eligible for. They can also help you find providers and specialists and coordinate transportation to doctor’s appointments.
    • Flu Prevention
      • Learn about how to protect yourself and your loved ones from the flu. Plus find information about where you can get your flu shot – at no cost to you.
    • Start Smart for Your Baby® Program
      • The Start Smart for Your Baby® program is for women who are going to have a baby, or just had, a baby. Learn how we can make this time easier for you and your family and how to manage you and your baby’s health.
    • Texas Health Steps
      • This program provides free checkups, vaccines and dental care to children birth through 20 years old who are enrolled in Medicaid.
