REQUIRED: Prior Authorization for Nursing Facility Add-On Therapy

Superior HealthPlan would like to remind STAR+PLUS Nursing Facility providers that Nursing Facility Add-On Therapy Services, for Dual Nursing Facility members require a prior authorization and should only be requested after all Medicare Part B therapy benefits have been exhausted.

Please Note: Effective April 1, 2024, Nursing Facility claims without an authorization will deny.
