Become a Contracted Provider
Superior HealthPlan is seeking qualified Long-Term Services and Supports (LTSS) providers to join our network and serve STAR+PLUS and Wellcare By Superior HealthPlan (Dual Align) members.
Our LTSS network helps members access the services, and supports they need to maintain independence, improve quality of life, and remain in their homes and communities whenever possible. By partnering with Superior, providers gain access to a broad network base, dedicated care coordination support, provider resources, and collaborative relationships with interdisciplinary care teams focused on delivering high-quality, person-centered care. Together, we work to improve health outcomes, support complex care needs, and expand access to essential LTSS services for some of the most vulnerable populations in Texas. We welcome providers who share our commitment to transforming the health of the communities we serve, one person at a time.

Together, we can expand access to quality LTSS services and support healthier communities across Texas.
Superior HealthPlan operates a toll-free Nurse Advice Line that can be found on Superior's Phone Directory webpage. Providers and members can call 24-hours-a-day, 7 days a week. The hotline is staffed with nurses who are knowledgeable about Medicaid and CHIP benefits and services. The nurses can respond to calls from providers and members seeking clinical information, guidance on specialty referrals or requests for specialty provider consultations. Nurses also have access to an on-call licensed behavioral health clinician 24-hours-a-day who can assist with crisis calls.
