Performant Notice for Home Health Providers

Superior HealthPlan has contracted Performant Recovery, Inc. to perform a review of claims submitted by home health providers. Reviews will be conducted to determine if the provider billed Superior appropriately for medically-necessary covered services. The review requirements are in accordance with the contract that exists between the provider and Superior.

When Performant conducts a review, the provider will receive a letter notifying them of the review. Please note:

  • The notification will list the applicable claims as well as the documentation that must be included with the clinical record and returned to Performant.
  • Clinical documentation must be received on or before the indicated due date outlined in the letter.
  • Failure to timely submit the requested records in their entirety may result in recovery of the previously paid claim.
  • Additional details for submitting requested documentation will be provided in the notification sent to the provider.
