REQUIRED: Nursing Facility Liability Insurance Rate Add-on Provider Attestation Due by July 31

As required by Title 1, Texas Administrative Code §355.312, all nursing facility providers must annually submit a liability insurance attestation to qualify for liability insurance rate add-ons. The Texas Health and Human Services Commission (HHSC) has published the SFY2026 Annual Required Attestation for Nursing Facility Liability Letter.

This is a reminder that an attestation must be submitted electronically through the HHSC Provider Finance Department (PFD) Nursing Facility Liability Insurance Attestation Portal. Submissions received via fax, mail, or email will NOT be accepted. The attestation period will take place until 5 p.m. July 31, 2025, for Fiscal year September 1, 2025, to August 31, 2026, at 5 p.m. CDT. Late submissions will not be accepted under any circumstances.
