HHSC Survey for Adult Foster Care Home Providers

The Texas Health and Human Services Commission (HHSC) began collecting survey responses from Adult Foster Care (AFC) home providers for State Fiscal Year (SFY) 2023 on Tuesday, September 3, 2024. The purpose of the survey is to better understand AFC in Texas. The survey responses will inform a review of the AFC reimbursement rate methodology and payment rates. HHSC is collecting the survey information in accordance with HHSC’s Biennial Fee Review, which requires HHSC to review all reimbursement methodologies and rates on a biennial basis.

Please note the following important information:

• To download the template for the survey, please visit the Adult Foster Care (AFC) Provider Survey section of the HHSC Provider Finance Department (PFD) Long-term Services and Supports (LTSS) website.
  • Completed surveys must be sent by email to PFD-LTSS@hhs.texas.gov.
  • All survey responses are due by 11:59 p.m. CDT on Friday, November 1, 2024.
