Allwell from Superior HealthPlan is proud to be your partner in care. We want to help your patients stay in control of their health and have the reliable coverage, service and care they can count on. Your patients have from October 15, 2020 until December 7, 2020, to sign up or change their Medicare plan during the Annual Enrollment Period.

We are ready to help your patients with any questions during the Annual Enrollment Period, so they can keep receiving your expert and compassionate care. Working together, we can make this Annual Enrollment Period easier and ensure your patients have access to the best care in 2021.

• Patient Questions - Please encourage patients with questions to visit Allwell.SuperiorHealthPlan.com, or call Member Services at 1-844-796-6811 (HMO) or 1-877-935-8023 (HMO SNP).
• Become an Allwell Provider - To join the Allwell from Superior HealthPlan Network, fill out the online form at Join our Network.
• Eligibility - Providers may check Allwell member eligibility on Superior’s Secure Provider Portal.
• Billing Questions - For Allwell billing questions, please reference the Allwell from Superior HealthPlan (Medicare) Provider Manual.
