Participate in Planning Your Care

A care management plan is a plan made just for you. It talks about your health needs, lists your goals, shows the services you will receive, and explains who will help you and how often. This plan is made with you, not for you. Superior’s Care Managers will guide you in updating or finalizing your plan. You can ask questions and decline services you do not want. Your plan can change if your needs change.
Why You Should Be Involved
You know your body best. You know what works for you and what does not. When you share your thoughts, your care team can help you better. You can contribute to your care plan by:
- Sharing what is going well.
- Talking about what is difficult for you.
- Asking questions.
- Informing your Care Manager when things change.
Setting Simple Health Goals
Goals are things that matter to you and help improve your daily life. They should be easy to reach. Small steps can make a big difference. Some examples of goals are:
- Managing health conditions or feeling better.
- Staying safe at home.
- Improving mobility or completing daily tasks.
- Reducing stress for you or your caregiver.
We are Here to Help
You are not alone. Your care team is here to help you. If your condition changes, you experience a new injury, your living situation changes, you need help finding a provider, or updating your care plan let us know. We can change your plan to better meet your needs. You can contact your Care Manager or Service Coordinator Monday through Friday between 8:00 a.m. - 5:00 p.m. (CST). Nurses are available 24 hours a day, 7 days a week through Superior’s nurse advice line at the number on the back of your member ID card.
