NEW: Provider Representative Contact Form and Network Status Inquiry Form

Superior HealthPlan providers have two new digital forms available for submitting status inquiry requests to our Provider Engagement teams. Using these forms may help expedite requests by gathering all required information upfront. This allows Provider Representatives to assist as soon as the forms are received and eliminates any potential follow-up emails requesting additional information.

Provider Representative Contact Form

Access the form by going to the Superior website and clicking Provider Resources from the left tool bar then selecting Find My Provider Representative then Provider Representative Contact Form.  

The Provider Representative Contact Form allows providers to request the following:

  • Assistance Finding Provider Representative    
  • Contract Termination Request
    • Termination of an individual from a group contract
    • Termination of group contract
    • Termination of specific product
  • Update Demographic Information
  • Claim Project Status
  • Negative Balance Request
  • Explanation of Payment Request

Network Status Inquiry Form

Access the form by going to the Superior website and clicking Network Request or Update from the left tool bar then selecting Provider Representative Contact Form.

The Network Status Inquiry Form allows providers to request status updates on:

  • Contracting and Initial Credentialing Status
  • Recredentialing Status
