Effective 10/1: Collaborative Care Model Rate Change

Superior HealthPlan would like to inform providers participating in Collaborative Care Model (CoCM) about the reimbursement rate change for 99494 beginning October 1, 2023.

The rate will increase from $50.28 to $58.28 for an Office Location and from $34.81 to $42.81 for a Facility Location.

As a reminder, the CoCM is an integrated modality that provides patients with medical and behavioral health care in a primary care setting. An increasing number of primary care settings are incorporating behavioral health services. Benefits of CoCM include:

• Improved patient and provider satisfaction.
• Reduction in health care costs.
• Reduction to possible health disparities, in access to behavioral health.
