New Requirement for Minor Home Modifications

Superior HealthPlan is committed to ensuring transparency, consistency and efficiency in the Minor Home Modifications (MHM) process for our members. To support this commitment, we are implementing a new requirement for itemized pricing in all MHM bid submissions, effective November 1, 2025.

Moving forward, all bid submissions must include a detailed itemization of costs. This applies to all projects, regardless of scope or value. MHM providers can use Superior’s MHM Bid Form (PDF) or their own bid form as long as it includes all required information from Superior’s Bid Form Template.

When providing information on the modification needed, please be sure to include:

  • Each Modification Item/Itemized Description – A clear explanation of each proposed improvement or modification.
    • Unit Cost per Item/Service – The cost for each material and/or labor component.
    • Quantity – The number of units per item or service being proposed.
    • Labor and Materials Breakdown – Labor and material costs should be listed separately where applicable.
    • Tax and Permit Fees – If applicable, include any taxes or local permit costs.
    • Total Cost per Line Item – The total cost for each item (unit cost × quantity).
  • Total Bid Amount (Project Total) – The total amount requested for the entire job.

Failure to submit bids with proper itemization may result in delays in authorization or the rejection of your proposal. Please refer to the Superior MHM Bid Form Example (PDF) for guidance on filling out the form.
