Asthma Help

Superior wants to make sure if you have asthma you get the support you need to live a normal life. Use the resources below to help you manage your asthma.

As a Superior member, you have access to Superior’s Asthma Care Management program. If your asthma is difficult to manage or you need assistance accessing benefits and services, we can help. Superior's Care Managers are registered nurses and licensed social workers who will work with you to provide support and answer questions. As your Care Manager, we can help schedule appointments, provide transportation assistance and find resources to help you get well and stay well. We can also help secure other behavioral and social services.

We can help find community services that will help improve your overall health. We will work with you to:

  • Identify and address your basic needs.
  • Locate benefits and support in your community.
  • Manage your care and resources.

As a Superior member, you have access to Superior's Asthma Disease Management program. If your asthma is mostly under control but you still need ongoing support from a health coach we can help. Disease Management connects you to a registered or certified respiratory therapist to help you manage your asthma symptoms. They can help you answer questions like:

  • What sets off my "asthma attacks?"
  • When do I need to take my asthma medicines?
  • How can I stop an “attack” before it starts?
  • Should I go to the hospital if I think I’m having trouble breathing?
  • What can I do to lower my chance of an "attack?"

Your Superior care team can answer questions about how to manage your asthma at home with the support of your doctors and other providers. Your care program is designed for your individual health needs. You can send a private message to your care team at any time. You'll know just what to do to self-manage your asthma at home.

findhelp is an online social service resource directory to help locate community supports such as food and nutrition, housing, education, and employment services. Enter your zip code and click on the search function to begin.
With access to more than 4,000 topics relating to health and medication, Krames Staywell Health Library allows you and your family the ability to find answers to most questions, both big and small. Simply click on the search function to begin.
You can find health and wellness educational books on asthma and much more. Materials are personalized for different age groups and are available in English and Spanish.
The Asthma Health Visit Form (PDF) can be used to help you ask the right questions when you or your child visit the doctor.
Controlling your asthma can save your life. Work with your provider to complete this Asthma Action Plan (PDF).
As a Superior member, you have access to additional Value-added Services to help you control your asthma symptoms. Members can get extra benefits and services in addition to your regular benefits. These are called Value-added Services (VAS). Read more about your VAS benefits and call Member Services to learn how to get these benefits for you/your child.
As a Superior VAS, members get money to use towards commonly used over-the-counter (OTC) items, per member, per quarter. Learn more about the over-the-counter mail-order program, available items, pricing and ordering instructions.
