STAR+PLUS Medicaid Plan

As a Superior STAR+PLUS member, you can get many great services in addition to your regular Medicaid benefits. These are called Value-added Services.* From extra vision care to cell phones and home-delivered meals, Superior is proud to offer many extra services to help keep our members healthy. • 3-month subscription for Weight Watchers^® online program. Members must have a BMI value of 30 or higher and a primary diagnosis of diabetes, prediabetes, hypertension or heart disease to qualify.
• 24-hour emergency response services for non-HCBS Waiver and non-CFC members, for up to 6 months following discharge from a hospital or nursing facility.
• $50 reward for members upon attending and graduating from an 8-week fall prevention program called, A Matter of Balance.^
  Extra help getting a ride to classes is available through Superior's Medicaid Ride program.
• Phone and extended minutes. A Connections Plus pre-programmed phone with unlimited talk and text for qualifying members enrolled in Care Management.
• Home-delivered meals. Non-HCBS Waiver members who do not reside in an ICF-IID residential home, have access to 10 home-delivered prepared meals, when discharging from a hospital or nursing facility.
• In-home respite services. Non-HCBS Waiver members with certain complex and chronic conditions have access to up to 16 hours of in-home respite services each year.
• General Educational Development (GED) preparatory materials and resources to help pass the GED test for members with an IDD diagnosis.
• Online resources. Online mental health and nicotine recovery resources.
• Extra help getting a ride. Members may seek transportation assistance through Superior's Medicaid Ride program for dental, health, or behavioral health appointments along with safety and education classes, and more. Members can get a ride to local benefits office.
• Pregnancy, prenatal and postpartum related rewards. $25 reward and educational materials for pregnant members attending a virtual educational baby shower and completing a Notification of Pregnancy (NOP) form.^ Members can also earn up to an additional $150 My Health Pays Rewards® for healthy activities related to your pregnancy and delivery.^
• Up to 4 nutritional visits per year with a registered dietitian.
  
• Up to $100 Intellectual and Developmental Disabilities (IDD) camp allowance to promote opportunity and increased independence.
• An online “caring community” enabling members to engage friends and family as it relates to their healing or healthcare journey.
• Drug store $0 co-pay on all Part-D formulary-approved prescription drugs.*******
• 2 pest repeller plug-ins to help control asthma and chronic obstructive pulmonary disease (COPD) symptoms. Members must be enrolled in Asthma or COPD Disease Management for 60 continuous days.
• Nursing facility welcome kit. Welcome kit includes personal items to help with transition to living in a long-term care facility.

Note: The Value-added Services listed below do not apply to dual eligible members. 

• $20 reward every 6 months for members with diabetes who complete a blood sugar test (HbA1c) with a result less than 8.^
• $20 reward for members who go to a follow-up doctor visit within 7 days of a substance use disorder ER visit.^
• $20 reward for members who complete a follow-up appointment within 7 days of leaving the hospital for an inpatient behavioral health or substance abuse stay.^
• $50 reward for female members at average risk for cervical cancer, who complete a recommended cervical cancer screening.**^
• Extra dental benefits. 
  • Members may get up to $750***, $500**** or $250***** annually for exams, x-rays, cleanings, fluoride treatments and simple extractions.****
  • Dental care kit to keep teeth and gums healthy.*****
  • Extra help getting a ride to dental VAS visits available through Superior's Medicaid Ride program.
• Joy for All™ battery-operated plush companion pet for members enrolled in MIND at Home or Cognitive Adaptation Training Care Management for at least 60 days. Members must have a diagnosis of Dementia or Alzheimer’s.******
• Help for members with Asthma. Members enrolled in Asthma Disease Management for 60 days get an allergy free mattress cover and pillowcase.    
• Extra vision services. Members may get up to $150 for choice of upgraded eyeglass frames and lenses or contact lenses not covered by Medicaid each year.    
• Up to $120 yearly allowance ($30 per quarter) to use for commonly-used over-the-counter items that are not covered by Medicaid.
• 3-month YMCA membership for members with high blood pressure, a history of hypertension, and a BMI of 30 or greater.********
• Personalized medical identification bracelet for members with an Intellectual and Developmental Disabilities (IDD) diagnosis.*********
