Equality Health Deepens Value-Based Care Offering with Providers in Louisiana

Equality Health, a national leader in value-based care with its Medicaid-first model, and Louisiana Healthcare Connections, a managed care organization serving members across Louisiana and a company of Centene Corporation (“Centene”) [NYSE: CNC], today announced plans to support providers in advancing value-based care (VBC) in the state. The collaboration is aimed at closing health equity gaps for Medicaid members and is an expansion of the relationship Equality Health has developed with Superior HealthPlan ("Superior”) to support providers in Texas, beginning in 2021.

Equality Health, Louisiana Healthcare Connections and Superior have aligned missions to address health inequities among diverse and historically marginalized populations at a local level.

Equality Health is helping transform the current state of healthcare by aligning, engaging, and empowering providers and health plans to focus their work on the members and communities that need it most. For example, in Texas, Superior and Equality Health have collaborated to improve the level of quality care accessible to Medicaid members. This includes a 5% increase in follow-up care for children prescribed attention-deficit/hyperactivity disorder medication as well as 4% increases in cervical cancer screenings, immunizations for adolescents, and blood pressure control for diabetic patients.

Since 2022, Equality Health has worked closely with Superior to expand access to critical healthcare services in Uvalde, Texas, a community impacted by a school shooting. This includes a partnership with the Equality Health Foundation to help develop Uvalde’s new health and wellness center.

By fusing technology, coaching, clinical expertise, financial support and a deep understanding of VBC, Equality Health helps improve whole-person care. According to a recent report on the VBC enabler landscape, “Equality Health’s focus on Medicaid beneficiaries gives a unique shape to its care model... successful population health management within Medicaid depends heavily on risk stratification, prioritizing interventions, and matching those interventions to the level of need.”
