Texas Medicaid Formulary Update: Xifaxan (rifaximin) Removal

Superior HealthPlan Texas Medicaid is notifying providers that Xifaxan (rifaximin) has been removed from the formulary due to termination of the manufacturer’s federal Medicaid drug rebate agreement.
Effective April 17, 2026, pharmacy claims for Xifaxan (rifaximin) will reject because the product is non-formulary. There are no other rifaximin products available with a federal rebate agreement. We are requesting providers refer your patients to the program assistance resources with the manufacturer.
