Superior HealthPlan would like to inform providers that the following credentialing voicemail line is no longer in use and will not be monitored going forward: 1-800-820-5686.

To ensure any questions are addressed promptly, please direct all credentialing inquires to Superior’s credentialing email at: Credentialing@SuperiorHealthPlan.com.

To help ensure Superior can assist you as quickly as possible, please include the provider's name, NPI, and Tax ID in your email, along with a brief description of the issue or request.

For all other inquiries, please submit a Network Status Inquiry Form.
