Durable Medical Equipment (DME) Provider Database

Effective June 1, 2022 Superior HealthPlan launched a Durable Medical Equipment (DME) Provider Database form to obtain information from Superior’s contracted DME providers. All in-network DME providers are encouraged to complete the form which can be found at Durable Medical Equipment (DME) Provider Database. The purpose of the form is to:

  • Allow in-network DME providers to specify the services or supplies dispensed.
  • Assist Superior Service Coordinators and Customer Service Representatives in efficiently locating appropriate services and supplies for Superior members.

Information submitted to the database will be subject to determination of medical necessity and/or prior authorization.

Please Note: Individuals completing the form must certify that they are an authorized affiliate of the DME provider for whom the form is being completed.
