Your Next Step After a Hospital Visit

Schedule a follow-up appointment within 7 days after a hospital stay, Emergency Room (ER) visit, or medical procedure. Whether that be with a specialist or your primary care provider (PCP), a follow‑up appointment helps them check your progress and prevent other health problems.

At your follow-up appointment your doctor might:

  1. Review your medications with you. This includes any new medications you might be taking after a hospital or ER visit.
  2. Go over test results.
  3. Talk about next steps for your health.

You play a part in your follow-up care. Here are a few ways you can make the most of your follow-up appointment:

  1. Tell your doctor how you’ve been feeling and any health concerns you may have.
  2. Bring a list of your medications and any test results from your previous hospital visit so that you can go over them with your doctor.
  3. Ask questions. If your doctor prescribes any new medications or wants you to have more tests done, make sure you understand what they are for and what to expect.
