Effective 1/1/26: Medicare-Medicaid Plan (MMP) is transitioning to an aligned Dual Special Needs Plan

Beginning January 1, 2026, Superior HealthPlan will launch an aligned Dual Special Needs Plan (D-SNP) in Texas. As directed by CMS, our current STAR+PLUS Medicare-Medicaid Plan (MMP), will sunset on December 31, 2025. Members will automatically be transitioned into the new aligned D-SNP, called Wellcare By Superior HealthPlan.


Like MMP, this new plan is designed for individuals who qualify for both Medicare and Medicaid, allowing their benefits to be coordinated and managed by a single healthcare organization.

The Texas Health and Human Services Commission (HHSC) is also hosting an upcoming webinar on November 20, 2025 to learn about aligning enrollments between an Integrated Dual Eligible Special Needs Plan (D-SNP) and a STAR+PLUS plan for members who receive Medicare and Medicaid services.
