My Health Pays® Rewards

Get rewarded for focusing on your health!

Earn My Health Pays® rewards when you complete healthy activities. Use Your My Health Pays rewards to help pay for:

  • Utilities
  • Transportation
  • Telecommunications
  • Childcare services
  • Education
  • Rent

Or, you can use them to:

  • Shop at Walmart for everyday items**

**This card may not be used to buy alcohol, tobacco, or firearms products. At this time, you cannot use rewards at walmart.com – you must visit a store.

Earn Extra Money with My Health Pays®   

Healthy Activities Eligible For Rewards

  • $75 for prenatal visit within the first trimester or 42 days of enrollment with Superior.
  • $50 for postpartum visit within 7-84 days of delivery.
  • $25 for attending a Superior hosted virtual baby shower and completing a Notification of Pregnancy (NOP) form.

Restrictions and limitations may apply. Eligible members may receive up to a maximum of $150 per year across My Health Pays® rewards.

  • $75 for prenatal visit within the first trimester or 42 days of enrollment with Superior.
  • $50 for postpartum visit within 7-84 days of delivery.

Restrictions and limitations may apply. Eligible members may receive up to a maximum of $150 per year across My Health Pays® rewards.

  • $75 for prenatal visit within the first trimester or 42 days of enrollment with Superior.
  • $50 for postpartum visit within 7-84 days of delivery.
  • $25 for attending a Superior hosted virtual baby shower and completing a Notification of Pregnancy (NOP) form.
  • $25 for completing all recommended flu vaccines before member’s second birthday. (6 months - 2 years.)

Restrictions and limitations may apply. Eligible members may receive up to a maximum of $150 per year across My Health Pays® rewards.

  • $100 for prenatal visit within the first trimester or 42 days of enrollment with Superior.
  • $50 for postpartum visit within 7-84 days of delivery.
  • $25 for attending a Superior hosted virtual educational baby shower.
  • $20 for completing a follow-up visit within 7 days of an emergency room visit for a substance use disorder. (One per year.)*
  • $20 for completing a follow-up visit within 7 days of an inpatient hospital discharge for a mental health or substance abuse episode. (One per year.)*
  • $20 for members with diabetes who receive a blood sugar test with a result less than 8. (Limited to two $20 rewards per year, every six months.)*
  • $50 for completing a recommended cervical cancer screening. Ages 21-64. Females only. (One per year.)*
  • $50 for completing “A Matter of Balance” 8-week fall prevention program. (Limited to one per member lifetime.)

*Value-added Service also available to STAR+PLUS Nursing Facility members.

  • $25 for completing a Texas Health Steps* checkup. (One per year.)
  • $30 for completing an initial or 6-month dental checkup. (One per year.)
  • $20 for receiving a flu shot during flu season. 
  • $50 for a postpartum visit within 7-84 days of delivery. (Females only. All ages.)
  • $20 for completing an annual cervical cancer exam (One per year, females only.)
  • $50 grant for education, utilities, transportation, and more. (One per year.)

*Also known as Early and Periodic Screening Diagnosis and Treatment (EPSDT).

  • $10 for an annual flu vaccine. (One per year.)
  • $120 for completing 6 Texas Health Steps checkups on schedule. Ages birth to 15 months.
  • $20 for an Annual Well Woman Exam. Ages 18 to 20 years. Females only. (One per year.)
  • $25 for completing a yearly Texas Health Steps checkup. Ages 2 to 20 years.
  • $15 for completing a transition assessment. Ages 16 to 20 years.
  • $15 for completing and submitting guardianship documents. Ages 18 to 20 years.
  • $25 for participating in a wellness activity, non-school sports program or in a community physical activity. Ages 11 to 16 years. (Nueces and Hidalgo service delivery areas only.)
  • $25 for completing your annual STAR Kids Screening and Assessment, returning a signed Individual Service Plan and being up to date with your Texas Health Steps check ups following 90-days of continuous enrollment. (One per year. Non-MDCP members in MRSA West and Lubbock service delivery areas only.)
  • $16 for obtaining a Texas identification card. Ages 16 to 20 years.(Limited to one per member lifetime.)
  • $30 reward ($10 per visit) for completing a follow-up doctor visit within 7 days of leaving the hospital for an inpatient behavioral health or substance abuse stay. Ages 6-20 years.

Pregnant members can receive rewards for completing these activities following confirmation of the visit:

  • $25 for attending a Superior hosted virtual educational baby shower.
  • $20 for a prenatal visit within your first trimester or 42 days of Superior enrollment.
  • $20 for your 3rd prenatal visit.
  • $20 for your 6th prenatal visit.
  • $20 for your 9th prenatal visit.
  • $20 for your postpartum doctor visit within 7-84 days of delivery.

Please note: Beginning January 1, 2027, members must enroll in the My Health Pays® program to earn rewards. Call Member Services using Superior's Phone Directory to enroll. If you complete eligible reward activities on or after September 1, 2026, before enrollment begins on January 1, 2027, you will still receive those rewards. Once you enroll, any eligible rewards earned before enrollment will be added to your My Health Pays® card.

Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays® Visa® Prepaid Card will be mailed to you.

This card is issued by The Bancorp Bank, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Card cannot be used everywhere Visa debit cards are accepted. See Cardholder Agreement for complete usage restrictions.

Frequently Asked Questions

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you.  Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact Member Services.

Once you receive your My Health Pays® rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation. 

To check your balance, login to your Secure Portal account.

Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
