Long Term Services and Supports (LTSS)

Every year, millions of people of all ages use and rely on Long-Term Services and Supports (LTSS), from meal delivery to self-directed personal care services. At Superior HealthPlan, we’re here to support you with long-term care services and supports.

What are LTSS?

LTSS are benefits for members who need a more hands-on and customized approach to care. These benefits help members stay safe and independent in their home or community.

The Value of LTSS

Some of the ways LTSS help take care of members include but are not limited to: 

Members are assigned a Service Coordinator who can help with their needs. This includes coordinating LTSS and creating a person-centered service plan. They are also the main point of contact for any questions members may have about their benefits. Learn more on our Care Management & Service Coordination webpage.

A personal care attendant is someone who helps with everyday tasks such as meal prep, cleaning, bathing, and getting dressed.

Day Activity and Health Services (DAHS) centers have on-site trained staff who offer health support, medication supervision, and rehabilitation. In addition, they provide opportunities for members to socialize and engage in various activities.

Assisted living centers provide members with apartment-style living and offers services like meal delivery, personal care services, and 24/7 staff availability.

Self-directed care services give members more of a choice and control over some of the LTSS they receive and who provides them. Visit our Consumer Directed Services webpage to learn more.

Home meal delivery offers balanced, prepared meals for members. This helps make sure regular nutrition is available when cooking or grocery shopping is challenging.

Respite care gives unpaid caregivers a short break while someone else supports their loved one at home or in a facility.

  • Employment Assistance helps members find paid employment in the community.
  • Supported Employment helps members succeed at their job tasks and keep their job.

Emergency response devices provide fast access to emergency assistance, helping people stay safe and independent at home.

Additional Services and Benefits

Here are more ways Superior supports our members: 

Superior's Find a Provider tool lets you search through our directory for LTSS providers in your area. You can also search for Primary Care Providers (PCPs), physicians, hospitals, drug stores and other health-care providers that are here to help you achieve your health care goals.

Members can talk to a nurse 24 hours a day, 7 days a week through Superior’s nurse advice line at no cost. By calling the nurse advice line, you could save time and a trip to the ER. You can get help with:  Answering your health questions.

  • Talking about your health concerns, treatment options, and medications.
  • Discussing if you can wait to see your doctor. 
  • Locating an Urgent Care clinic or after-hours clinic. 
  • Evaluating if you need to go to the emergency room. 
  • And more.

If you have an emergency or crisis, call 911 or visit the nearest hospital or emergency room.

A program that can help members get commonly used over-the-counter (OTC) items like medications, household and health items. These items may be available in stores or by delivery. Visit our OTC webpage for a full list of items, to download the catalog, and for ordering instructions.

Superior’s Medical Ride Program provides transportation to non-emergency health care appointments for members who have no other transportation options. Visit our Transportation webpage to learn more.

Get 24-hour access to doctors or therapists at no cost, in the privacy of your home, your office, or on the go. Telehealth is available for non-emergency health issues like skin problems, sinus infections, or cold and flu symptoms. You can get medical advice, a diagnosis, or a prescription all by using your mobile device. Learn more about Superior’s Telehealth services.

We are proud to offer extra services to keep our members healthy. As a member, you are able to get extra benefits and services in addition to your regular benefits.
